Profiles: Importing a Group of Youth Profiles
There are two way to add a youth into your database:

- Importing a group of youth profiles
To import a group of youth profiles:
- Access “Profiles” on the left navigation bar
- Click “Import group”
- This will open an instruction window


- Click “Download template” to access the excel file template (1)
- Do not add or delete pre-populated column headers (this will cause your import to error)
- Fill in the spreadsheet with all youth profiles you would like to import
- All fields are required
- For the Identifier field, P4C recommends using an identifier already attached to that youth within your organization such as EHR, MRN, or Student ID
*Identifier is what the youth will need to enter to access and take a survey - Race, ethnicity and insurance fields must be chosen from pre-populated options
* If you do not capture this data for your youth population there are ‘other’ or ‘unknown’ options to select
- Save the file as a .csv
- Do this by changing the file format when saving your spreadsheet

- Save the .csv file to your computer
- Click “Choose File” in the instruction window (2)
- Locate your file
- Click “Import CSV” (3)
- You import will populate into the Profiles menu
